I was looking at the Era Gold V based on Fremer’s review where he made it seem like the greatest phono stage ever (high praise, indeed) but my concern is the fact that it came out almost 20 years ago. The price difference between the Era Gold and the Reflex seems well worth it for a decade newer technology. My question is Reflex M or Accession.

The Accession costs about 1/3 more than the Reflex M & VFM is important to GSP.
Quantifying sound quality improvements in terms of a percentage is a strange game and can be misleading. I thoroughly enjoy using both the Accession M & the Reflex M - the Reflex M with an Elevator EXP. If you can afford the Acc then it is probably the one to go for. Does anyone know how many iterations the Accession M has gone through? If I buy one used, will I be missing upgrades/mods if it's too old?

The zenner diode reference voltage was replaced by a resistive split within a year of launch. I think a unit dated after 2015 should meet your requirement. The only problem is identifying the serial number, which means you'd need to ask John C. We do it that way to ensure nobody cheats.
